Complex Full-Mouth Rehabilitation & Vertical Dimension Restoration

Patient Profile: 58-Year-Old Female

The Challenge: The patient presented with a heavily compromised dentition characterized by generalized attrition (wear) and active signs of enamel erosion. This extensive tooth loss had led to a collapsed bite and a severe anterior deep bite. Her functional and aesthetic foundation was further complicated by missing posterior teeth, non-restorable root stumps in the posterior maxilla (#17, #27), and extreme wear that threatened the pulpal vitality of several remaining teeth.

Our Solution:

Biological Foundation & Clearance: The treatment initiated with the atraumatic extraction of the hopeless root stumps (#17, #27), with a phased plan for their future replacement. To secure the structural foundation, expert endodontic therapy was performed on the most severely attrited teeth (#26, #33, #35, #44, and #45).
Neuromuscular Rehabilitation & VDO Optimization: To safely correct the severe deep bite and restore facial proportion, the patient’s bite was deprogrammed. We engineered a new, optimal Vertical Dimension of Occlusion (VDO) and transitioned the patient into full-arch provisional restorations. The patient functioned in these provisionals for a 3-week diagnostic phase to thoroughly verify joint comfort, masticatory function, and aesthetic integration before finalizing the treatment.
Definitive Structural Restoration: Following the successful adaptation to her new bite, the definitive rehabilitation was executed using premium, multilayered full-contour zirconia across the dentition. This elite material selection provides maximum monolithic strength to protect against future wear and erosion, while delivering a highly vibrant, natural, and comprehensively rejuvenated smile.
